The reason you can`t play high is because you`re switching between instruments all the time. The trumpet and trombone both have differeny size mouthpieces, so your lips have to adjust between the two. You can`t really make it so you can switch between the two perfectly, but you can get your lips to adjust faster. I play both as well and after playing the trombone my trumpet chops aren`t to great.

What i really think happens is that both instruments aquire two different embouchures. After playing trombone your muscles are set on thinking trombone, so when you switch to trumpet your muscles think youre going to play trombone. You have to learn how to tell your chops to calm down and set the right embouchure. That or try to find one ombouchure that works for all. You should check out james morrison on trumpetstuff.com. Look at the two horn clip and you will be able to see this guy switch between both horns with a great sound. I suggest watching how he plays, look at his face, what he does before he puts the horn to his face, the way he stands. Body language is a great key to figuring out how some musicians create such beautiful music.
